Zusammenfassung
Während die Überwachung hämodynamischer und pulmonaler Funktionen bei Intensivpatienten heute als selbstverständlich gilt, beschränkt sich die Überwachung des Zerebrums vielerorts noch auf eine gelegentliche Pupillenkontrolle, Auslösung von Reflexen sowie die Bewußtseinsüberwachung. Es ist somit nicht verwunderlich, daß die Mortalität schwerer Schädel-Hirn-Traumen trotz verbesserter intensivtherapeutischer Möglichkeiten in den letzten 30 Jahren kaum abgenommen hat [25]. Saul u. Ducker [55] konnten anschaulich nachweisen, daß durch eine konsequente Senkung des intrakraniellen Druckes bei schwerem Schädel-Hirn-Trauma die Mortalität von 46 auf 28% reduziert werden konnte. Voraussetzung hierfür ist jedoch, daß einmal eine intrakranielle Druckerhöhung mit geeigneten Maßnahmen erkannt wird und andererseits dies frühestmöglich geschieht. Beide Forderungen ergeben sich aus der Erkenntnis, daß sich die Überlebenschancen, v. a. das Überleben ohne schwere Dauerschäden, mit zunehmendem intrakraniellen Druck drastisch verringern. Während bei einem intrakraniellen Druck von 20 mm Hg eine Mortalität um 20% zu erwarten ist, muß bei länger anhaltenden Hirndrucksteigerungen über 50 mm Hg mit der irreversiblen Schädigung des Gehirns gerechnet werden. Die Mortalität ist dabei jedoch weniger abhängig von der Absoluthöhe des intrakraniellen Druckes, sondern von der Geschwindigkeit, mit der der intrakranielle Druck ansteigt.
