Abstract
Postoperative chylothorax resulting from injury to the thoracic duct (Fig. 35a) is a relatively rare, but nevertheless well-known problem following operations on the aorta, heart, lungs and oesophagus. The best therapeutic procedure for this situation is a matter of controversy. Conservative methods usually involve drainage and parenteral feeding. Despite substitution there is the nevertheless a loss of proteins, fat-soluble vitamins, electrolytes and lymphocytes [12]. In approximately 50% of conservatively treated cases, a spontaneous leak closure is achieved after varying periods of time, otherwise a surgical intervention is necessary [51,116].
