Zusammenfassung
Schock, Trauma, Verbrennungen und ausgedehnte chirurgische Eingriffe führen über eine Aktivierung von Makrophagen zu einer erhöhten Synthese und Sekretion von Tumornekrosefaktor-α (TNF-α) [1, 2]. Zahlreiche in vitro Studien [3] belegen, daß TNF-α zu einer signifikanten Steigerung der Prostaglandinsynthese und im besonderen von PGE2 führt. Die erhöhte Freisetzung von PGE2 aus Makrophagen könnte für die nach Trauma und Verbrennung auftretende Immunsuppression eine entscheidende Rolle spielen, da PGE2 zu einer massiven Hemmung des zellulären und humoralen Immunsystems führt. Aus diesen Ergebnissen läßt sich die Hypothese ableiten, daß TNF-α indirekt über die erhöhte Freisetzung von PGE2 für die posttraumatische Immunsuppression verantwortlich sein könnte. Es war das Ziel dieser Studie, den Einfluß von rekombinantem TNF-α (rTNF-α) auf Funktionen des zellulären Immunsystems zu untersuchen. Um Effekte von TNF-α auf kardiopulmonale und metabolische Funktionen auszuschließen, wurde eine niedrige Dosierung von rTNF-α verwendet.
