Zusammenfassung
Prinzipiell lassen sich kolorektale Anastomosen manuell mit Nähten in verschiedenen Variationen und mechanisch mit Klammern (Fazio 1991; Thiede et al. 1987; Wolmark 1987) oder Kompressionsringen herstellen (Hardy et al. 1987; Thiede et al. 1991). Vergleichsuntersuchungen zwischen Handnahttechniken, Staplernahttechniken und Kompressionsanastomosen bei vergleichbarer Lokalisation zeigen bei rechtsseitigen und linksseitigen Kolonresektionen in kontrollierten Studien prinzipiell keine wesentlichen Unterschiede in verschiedenen perioperativen Zielkriterien, wobei die Leckagerate das am stärksten beobachtete Zielkriterium darstellt (Cahill et al. 1989; Corman et al. 1989; Bubrick et al. 1991). Bei Beachtung des Zielkriteriums, Spätstenoseentwicklung, gibt es jedoch Hinweise auf eine häufiger bei Klammernahtgeräten einsetzende Spätstenose (Dziki et al. 1991; Luchtefeld et al. 1989) als bei den anderen Techniken mit voll absorbierbaren Nahthilfsmitteln, wie z.B. den biofragmentierbaren Kompressionsringen (Schubert et al. 1991). Unsere Arbeitsgruppe hat sich in Studien seit 1979 mit Stapleranastomosen und seit 1989 mit biofragmentierbaren Kompressionsringen, u.a. im kolorektalen Bereich, beschäftigt. Hier sollen die Ergebnisse der Studien mit Staplernähten vorgestellt und analysiert werden (Tabelle 1: Kontrollierte Studie, technische Durchführung 1979–1982, n = 60; prospektive Studie, technische Durchführung 1983–1988, n = 970).
