Zusammenfassung
Das konfokale Untersuchungsprinzip des Laser Tomographic Scanners (LTS) erlaubt neue Papillenparameter zu errechnen, die erstmals dazu verwendet werden, die Steilheit der Exkavationsränder zu quantifizieren.
153 Augen von 82 Patienten der Diagnosegruppen Gesund, okuläre Hypertension, Glaucoma chronicum simplex und Glaukom ohne Hochdruck werden mit dem Laser Tomographie Scanner (LTS) und dem Octopus Perimeter 201 untersucht.
Geprüft wurde, ob sich die Diagnosegruppen unterscheiden: 1. In der Tiefe der Papillenexkavation, 2. in der Form der Papillenexkavation und 3. in den Papillenparametern Exkavationsvolumen, Exkavationsfläche und Verhältnis Exkavationsfläche zu Papillengesamtfläche.
Der Vergleich erfolgte bei Augen mit Glaucoma chronicum simplex und Glaukom ohne Hochdruck bei gleichem Stadium des Gesichtsfeldausfalls.

Augen mit Glaukom ohne Hochdruck zeigten im Vergleich zu Augen mit Glaucoma chronicum simplex bei gleichem Stadium des Gesichtsfeldausfalls

— eine kleinere maximale und mittlere Tiefe der Papillenexkavation in allen Stadien mit Ausnahme des Stadium 1.

— signifikant steilere Exkavationsränder und einen flacheren Exkavationsboden im Stadium 1 und 2. In den fortgeschrittenen Stadien 3 bis 5 bestehen keine Unterschiede in der Steilheit der Exkavationsränder und der Form des Exkavationsbodens.

— im Stadium 1 ein signifikant größeres Exkavationsvolumen und eine signifikant größere Exkavationsfläche.

— im Stadium 1 bereits steile Exkavationsränder, während Augen mit Glaucoma chronicum simplex erst im Stadium 2 steile Exkavationsränder zeigen, das bedeutet: Ein Erkrankungsstadium später.

Gesunde Augen, Augen mit okulärer Hypertension und Augen mit Glaucoma chronicum simplex Stadium 1 zeigten keine signifkanten Unterschiede in der Steilheit der Exkavationsränder und der Form des Exkavationsbodens.
Augen mit okulärer Hypertension zeigten gegenüber gesunden Augen ein signifikant größeres Exkavationsvolumen.
Zwischen Augen mit Glaucoma chronicum simplex und Glaukom ohne Hochdruck wurden von uns in früheren Studien Unterschiede in Lage und Tiefe der Gesichtsfeldausfälle, Unterschiede in der Größe der Papillenexkavation und Unterschiede in der Fläche der neuroretinalen Randzone bei gleichem Stadium des Gesichtsfeldausfalls gefunden. In der vorliegenden Arbeit wurden hinsichtlich der Morphologie der Papille mit Hilfe des konfokalen Untersuchungsprinzips nun auch Unterschiede in der Steilheit der Exkavationsränder belegt.
Diese Unterschiede in der Morphologie der Papille weisen auf Unterschiede in den Schädigungswegen und Risikofaktoren in den Anfangsstadien bei Glaucoma chronicum simplex und Glaukom ohne Hochdruck hin.
Die Therapie des Glaukoms ohne Hochdruck muß daher diese zusätzlichen Risikofaktoren berücksichtigen.
