Abstract

Circulating cytotoxic antibodies against human leukocyte antigens (anti-HLA antibodies) create problems in obtaining a negative leukocyte crossmatch when transplanting a patient. The clinical problems due to positive crossmatches are best known and have been most extensively studied in renal transplantation, in which poor graft prognosis and relatively frequent rejection episodes have been demonstrated after transplantation of such sensitized patients. The extensive experience with sensitized renal transplant candidates is particularly due to the possibility of keeping these patients on dialysis for years while waiting for a crossmatch-negative graft or a passive or actively induced decline in cytotoxic anti-HLA antibodies. In this chapter, we will concentrate on how to avoid formation of cytotoxic anti-HLA antibodies and what to do with a renal transplant candidate if such antibodies have been formed.
