Zusammenfassung

Eine verständliche Abneigung gegenüber einer per manenten Ileostomie kann die Zukunft eines an Colitis ulcerosa oder familiärer Polyposis Leidenden auf unterschiedliche Weise beeinflussen. Sie beeinflußt gewöhnlich sowohl die Patienten als auch deren Ärzte dahingehend, so lange wie möglich von einer Operation abzusehen, wobei das mit beiden Erkrankungen verbundene Risiko eingegangen wird. Gleichzeitig orientieren sich die Chirurgen an den Behandlungsprinzipien der Rektumerhaltung durch eine ileorektale Anastomose, womit gleichzeitig erkrankte Rektumschleimhaut erhalten bleibt, die bei Colitis ulcerosa schließlich zu einer sekundären Proktektomie führen und bei der Polyposis zum Ausgangspunkt einer malignen Veränderung werden kann.
