Zusammenfassung
Eine mögliche Strategie zur Entwicklung klinisch-psychologischer Behandlungsmaβnahmen besteht darin, die biologischen und psychologischen Wirkmechanismen einer effektiven medizinischen Behandlung zu studieren und die verhaltenspsychologischen Maβnahmen danach zu “modellieren”. So haben wir z.B. die Behandlung der Skoliose und Kyphose mit orthopädischen Stützkorsetten untersucht und dabei festgestellt, daß weniger passive Stützfunktionen von diesen Korsetten ausgeübt werden, als daβ sie vermutlich ihre Wirkung über einen lempsychologischen Mechanismus entfalten: bei Haltungsänderungen, die mit einer Verkrümmung der Wirbelsäule einhergehen, kommt es zum Auftreten kontingenter Schmerzreize durch verstärkten Kontakt bestimmter Körperteile mit dem Korsett. Diese Schmerzreize wirken als kontingente Bestrafung für die eingenommene Fehlhaltung, der die Patienten durch rasche Haltungskorrektur entgehen. Auf der Basis dieser Analyse wurde ein “psychologisches Korsett” entwickelt, das ohne Schmerzreize auskommt und über einen positiven Verstärkungsvorgang dieselben Effekte erzielt (Dworkin et al. 1985).
