Zusammenfassung
1931 beschrieb Alpers [3] erstmals ein Krankheitsbild mit Entwicklungsverzögerung, Epilepsie und Leberstörungen nach dem 1. Lebensjahr. Pathologisch-anatomisch fanden sich eine okzipital betonte Poliodystrophie mit Astrozytose und Neuronenverlust sowie eine fettige Degeneration mit Hepatozytennekrose und Übergang zur Leberzirrhose. Nach Scheffner et al. [9] entsteht bei Kindern in durchschnittlich 1: 5 000 Fällen eine meist letal verlaufende Leberschädigung unter einer Valproattherapie. Eine gesicherte Ursache hierfür ist nicht bekannt, das Risiko ist bei antikonvulsiver Polytherapie, einem Alter unter 2 Jahren und vorbestehenden neurologischen Symptomen höher [2]. Aufgrund von zwei Fallbeschreibungen wollen wir auf mögliche Zusammenhänge dieser beiden Krankheitsbilder hinweisen.
