Zusammenfassung
In einer prospektiv geplanten klinischen Studie wurden an zwei Patientenkollektiven vergleichbarer Altersstruktur (je 50 Patienten) die funktionellen Ergebnisse nach Implantation bifokaler diffraktiver Intraokularlinsen (Gruppe 1) und monofokaler Linsen (Gruppe 2) verglichen. Ca. 6 Wochen postoperativ betrug der Fernvisus mit bester Korrektur 0,8 ± 0,17 (Gruppe 1) bzw. 0,87 ± 0,21 (Gruppe 2); der Nahvisus betrug in Gruppe 1 (Fernkorrektur ohne zusätzliche Nahkorrektur) 0,76 ± 0,18 und in Gruppe 2 (mit Nahzusatz) 0,87 ± 0,19. 52% der Patienten mit bifokaler Intraokularlinse hatten einen unkorrigierten Nahvisus von >0,63. Bezüglich des postoperativen Astigmatismus war für beide Linsentypen die Phakoemulsifikation die bessere Operationsmethode: Der durchschnittliche Astigmatismus betrug bei den mittels Phakoemulsifikation operierten Augen 0,75 dpt (subjektiver Gesamtsastigmatismus) bzw. 0,94 dpt (keratometrisch ermittelter Astigmatismus); die mittels klassischer e.c.-Technik operierten Augen zeigten durchschnittliche Astigmatismen von 1,40 dpt bzw. 1,59 dpt. Trotz der z.T. sehr guten Akzeptanz bleibt die Indikation zur Implantation diffraktiver Intraokularlinsen begrenzt.
Summary
In a prospective planned clinical study we compared the functional results after implantation of bifocal diffractive intraocular lenses to those of monofocal lenses. 100 patients were divided in two groups (n = 50) of similar age structure (group 1 = bifocal, group 2 = monofocal). About 6 weeks after Operation the far distance visual acuity with best correction showed 0.8 ± 0.17 in group 1 and 0.87 ± 0.21 in group 2. The near distance visual acuity (without near addition) in group 1 was 0.76 ± 0.18, near distance acuity in group 2 (with near addition) was 0.87 ± 0.19. 52% of group 1 showed an uncorrected near distance acuity of 0.63 or more. As far as the postoperative astigmatism is concerned the method of phacoemulsification had advantages to classical extracapsular technique in both groups: The average astigmatism that resulted subjectively after phacoemulsification was 0.75 dpt, objective corneal astigmatism was 0.94 dpt. In e.c.-operated eyes we found an average astigmatism of 1.40 dpt and 1.59 dpt, respectively.
