Zusammenfassung
Es wird der Prototyp einer Sulkus-gestützten Hinterkammerlinse vorgestellt, die eine begrenzte Vorwärtsbewegung ihrer Optik ermöglicht und somit eine Zoom-Wirkung erzeugt, wenn der Durchmesser des Sulcus ciliaris mit der Kontraktion des Ziliarmuskels abnimmt. Auf die histologisch bewiesene verzögerte Alterung dessen meridionalen Anteils wird besonders hingewiesen. Die bis ins Alter anhaltende Funktionstüchtigkeit der meridionalen Partie könnte im Verein mit der elastischen Kraft der Zonulalamelle der Linsenkapsel zur Pseudoakkommodation auch mittels intrakapsulärer elastischer Weichlinse genutzt werden: Das vorgestellte Modell hat die Form einer Kalotte. Ihr peripherer Wulst spannt den elastischen Hohlring der Zonulalamelle aus und überträgt dessen Kräftespiel verlustarm auf die nach vorne gerichtete, verformbare, bikonvexe Optik. Die Linse ist zur Implantation nach voderer Kapsulorhexis gedacht. Vorteilhaft wäre wahrscheinlich ein Material, das erst nach seiner Implantation durch Hydrierung sein volles Volumen erreicht.
Summary
The prototype of special sulcus fixated posterior Chamber lens is introduced: A limited forward movement of the optical system allows a zoom effect as the circumference of the sulcus ciliaris diminishes, due to contraction of the ciliary muscle. Special attention is given to the histology of the ciliary muscle, which shows a delayed involution of its meridional part. The preserved functioning of this meridional part even in old age, together with the elastic power of the zonular lamella of the lens capsule could enable a pseudoaccomodation of an elastic soft lens that is fixated in the bag. The presented model is dome-shaped. Its circumference is given by a roll, slightly directed posteriorly, which expands the cavity of the elastic zonular lamella. With accomodation, the elastic power of this lamella compresses the soft lens which results in a slight forward movement and thickening of the central optical disc, in this way increasing the refractive power. It is proposed to implant the lens after anterior capsulorhexis. A lens material that could expand by hydration after implantation could be a special advantage. To prevent from fibrosis of the zonular lamella the surface of the peripheral roll should be coated appropiatly with a cell growth impeding substance. A central Yag Laser capsulotomy might be easily done on the extendet posterior capsule, as soon as it becomes necessary.
