Zusammenfassung

Komplikationen bei Lokalanästhesie sind in der Augenchirurgie auf anatomische und pharmakologische Gründe zurückzuführen. Anatomische Komplikationen hängen von den technischen Eigenschaften, d.h. der Anwendungsweise der verwendeten Lokalanästhesie ab. Mögliche Komplikationen sind venöse oder arterielle Blutungen in die Orbita, Läsionen des Nervus opticus, Perforationen des Bulbus oculi, Okklusionen der zentralen Gefäße durch offenes Trauma oder Kompression infolge Injektion von Anästhetika, kontralaterale Amaurosis auf subarachnoidalem Weg, okulokardialer Reflex mit anschließender Bradykardie, Dysrhythmie, a.-v.-Blockade und Herzstillstand. Pharmakologische Komplikationen können durch Lokalanästhetika oder Additiva wie Adrenalin oder Hyaluronidase bedingt sein. Allergische Reaktionen sowie toxische Einflüsse auf das zentrale Nerven- und Herz-Kreislauf-System sind möglich.

Complications during local anaesthesia in ophthalmological surgery result from anatomical and pharmacological reasons. Anatomical complications depend on technical properties of the local anaesthetics used, i.e., the form of application. Possible complications are venous and arterial bleeding into the orbita, lesions of the optical nerve and Perforation of the Ophthalmie bulbus, occlusions of the central vessels by direct trauma or compression by injected anaesthetic solutions, contralateral amaurosis by subarachnoidal route and the oculo-cardiac reflex, followed by bradycardia, dysrhythmias, a. v. blockade and cardiac arrest. Pharmacological complications may be due to local anaesthetics or due to additiva, like epinephrine or hyaluronidase. Allergie reactions, toxical influences on the central nervous and the cardiac systems are possible.
