Zusammenfassung
Im Rahmen einer prospektiven Studie wurden 23 nicht vorbehandelte Patienten mit metastasiertem Kolonkarzinom und meßbaren Tumorparametern mit 5-Fluorouracil (400 mg/m2 Tag 1–5) als 24-h-Dauerinfusion und d-L-Leucovorin (200 mg/m2 Tag 1–5) als Kurzinfusion behandelt. In vierwöchigem Abstand wurden bis zu sechs Zyklen appliziert. Zwei Patienten erreichten eine partielle Remission (PR) (9%). Bei sieben Patienten (30%) ließ sich ein stabiler Krankheitsverlauf (NC) dokumentieren. Insgesamt konnte bei 9 von 23 Patienten (39%) ein Ansprechen festgestellt werden. 14 von 23 Patienten (61%) zeigten eine Tumorprogression (PROG). Die mittlere progressionsfreie Überlebenszeit betrug 5,2 Monate bei einer mittleren Beobachtungszeit von neun Monaten. Die Therapiemodalität erwies sich als gut verträglich und wurde in der Regel ambulant durchgeführt. Es handelt sich bei dem hier vorgestellten Therapieschema um eine effektive und gut tolerable Behandlungsform des metastasierten Kolonkarzinoms. Nach vorläufiger Auswertung ist bzgl. der Ansprechraten die 5-FU/LV-Behandlung in dieser Form der 5-FU-Monotherapie im historischen Vergleich nicht überlegen, zeigt jedoch eine geringere Toxizität.
Abstract
Folinic acid combined with 5-fluorouracil continuous infusion in patients with metastatic colorectal carcinoma
In a phase II study 23 non-pretreated patients with measurable recurrent or metastatic carcinoma of the large bowel received a five-day continuous infusion of 5-fluorouracil, 400 mg/m2/day (5-FU-CI) and a two-hour infusion of folinic acid, 200 mg/m2/day (FA) for five days; this was repeated every four weeks for six months. Patients were evaluated for response and toxicity at the end of every treatment. Median follow-up was nine months. Toxicity of the schedule was mild leading to dose reduction in no case. No dose escalation was performed. In two patients partial remission (PR) could be achieved whereas in seven cases stable disease (NC) could be demonstrated. Time to disease progression was 5,2 months. In comparison to other reported schedules, our study demonstrates similar response rates with less toxicity.
