Zusammenfassung
Beginnt man eine Antidepressivabehandlung neu, dann ist in den ersten Tagen abhängig vom jeweiligen Präparat zunächst nur mit einer unspezifischen, sedierenden und entspannenden Wirkung zu rechnen, die in Einzelfällen aber durchaus auch schon zu einer wesentlichen Entlastung führen kann. Die eigentliche antidepressive Wirkung tritt in der Regel jedoch erst mit einer Latenz von 8–14 Tagen auf und führt dann zu einer allmählichen Besserung (Quitkin et al. 1984; Benkert u. Hippius 1986). Berücksichtigt man des weiteren, daß die Behandlung in der Regel einschleichend begonnen wird und es deshalb einige Tage dauert, bis ein wirksamer Serumspiegel erreicht ist, dann muß für eine Akutbehandlung, selbst bei optimalem Verlauf, mit mindestens 4 Wochen Behandlungszeit gerechnet werden. Wird eine Behandlung vor dieser Zeit beendet, was in der täglichen Praxis nicht selten ist (Linden 1986), dann kann nicht von einer Antide-pressiva-Regelbehandlung gesprochen werden. Bis dahin handelt es sich entweder um vorfristige Behandlungsabbrüche, evtl. Spontanremissionen oder sonstige therapeutische Wirkungen, die nicht mit der eigentlichen antidepressiven Wirkung identisch sind, wie beispielsweise Sedierung. Von daher ist es gerechtfertigt, jede Antidepressivabehandlung, die weniger als 4 Wochen gedauert hat, als Kurzzeittherapie zu bezeichnen und gesondert zu betrachten.
