Zusammenfassung
Es wird ein Überblick über die bei vollständiger Präparation der Gerüst-, Gelenk- und Muskelkompartimente des Larynx erhaltenen Befunde gegeben. Beste Informationen ergeben sich bei Untersuchung nach Fixierung; auch bei unfi-xierter Darstellung des Larynx sollten aber alle wesentlichen Teile begutachtet werden. Ausgangsmaterial waren 127 Tötungsdelikte mit Angriff gegen den Hals. Gegenüber nur retrospektiv erfaßten Fällen des eigenen Untersuchungsgutes wie gegenüber Literaturangaben steigt die Frequenz an Ring-, Schildknorpelplatten- und Schildknorpelunterhornfrakturen deutlich an. Das beobachtete Spektrum von Verletzungskombinationen an Kehlkopf und Zungenbein umfaßte 35 verschiedene Typen, von denen etwa die Hälfte jeweils nur einmal vorkam; 63 % aller Verletzungsmuster fanden sich in den 5 häufigsten Kombinationen, beim Erhängen waren es sogar 97 %. Von besonderem Gewicht können grobe Einblutungen der Kehlkopfmuskeln sein, wenn — wie in immerhin 15 Fällen -keine Gerüstverletzung vorliegt; bei jüngeren Personen mit elastischem Gerüst stehen derartige Quetschungsblutungen im diagnostischen Gewicht mit Gerüstläsionen zumindest auf einer Ebene.
Summary
Prospective analysis of laryngeal findings in 127 cases of homicidal strangulation (mostly by ligature and/or by hand) over more than 5 years has been carried out. Preparation of the larynx was carried out after fixation and involved complete examination of: the hyoid bone; thyroid and cricoid cartilage; the laryngeal articulations; the inner and outer laryngeal muscles; and the laryngeal mucosa. The results show an increased incidence of the following fractures, visible only after specific search: fractures of the thyroid plates (from 6 % to 17 %); of the thyroid lower horns (from 4 % to 9 %) and of cricoid cartilage (from 6 % to 16 %). The frequency of skeletal fractures in all cases amounted to 68% (in manual strangulation 79 %, and in strangulation by ligature alone 43 %). A total of 35 various combinations of fractures of the different parts were found, most of them only once; the five most frequent combinations (lacerations of upper thyroid and/or major hyoid horns) accounted for 63 % of all cases with fractures. Although the amount of undamaged laryngeal cartilage decreases with increasing age, severe injuries were found in 10% to 30% of cases in all stages of life (except children). Bleeding into the cavity or the capsule of the laryngeal articulations was found in more than 50% of manual strangulations compared with 15 % of strangulations by ligature. Haemorrhages into the laryngeal muscles (especially into the vocal folds or the PCA muscles) are a common finding; they can be a result of congestion or (very importantly) of direct compression of the larynx. Large haemorrhages can occur without skeletal fractures (15 cases in this material); they then become a valuable finding for the expert. Combining all the described lesions, a completely undamaged larynx was found in only 5% of all cases; all these victims were killed by means of strangulation by ligature alone.
