Zusammenfassung

Ventrikuläre Herzrhythmusstörungen finden sich häufig sowohl bei hypertrophen [15] als auch bei dilatativen Kardiomyopathien [25]. Während der direkte Zusammenhang zwischen ventrikulären Herzrhythmusstörungen in Form von nichtanhaltenden Kammertachykardien und dem plötzlichen Herztod bei der hypertrophen Kardiomyopathie gesichert ist [22, 23], bleibt ein derartiger Zusammenhang bei der idiopathischen dilatativen Kardiomyopathie umstritten. Es ist sicher gerechtfertigt, bei den meisten Kardiomyopathien davon auszugehen, daß Rhythmusstörungen Folge der Herzmuskelerkrankung sind und also ein sekundäres Problem, ein Symptom der Erkrankung darstellen. Es gibt jedoch auch Hinweise, daß permanent bestehende supraventrikuläre wie auch ventrikuläre Herzrhythmusstörungen per se zu einer Verschlechterung der linksventrikulären Funktion führen können, also das primäre Problem darstellen. In diesem Fall spricht man von einer tachykardieinduzierten Kardiomyopathie [6].
