Zusammenfassung
Bei 20 Patienten im Alter von 31–65 Jahren mit postrheumatischer Mitralstenose wurde der Versuch einer transluminalen Ballondilatation unternommen. Der Bal-lonkatheter wurde transseptal oder retrograd eingeführt. In zwei Fällen entstand eine hämodynamisch unbedeutende Mitralinsuffizienz. Bei einer Patientin kam es nach transseptalem Einführen des Baiionkatheters zu einem bleibenden Vorhofseptumdefekt. In einem Fall entstand eine schwere, operationsbedürftige Mitralinsuffizienz. Andere ernste Komplikationen traten nicht auf. Bei drei Patienten konnte aus technischen Gründen keine Dilatation durchgeführt werden. Bei 16 der 20 Patienten verlief der Eingriff erfolgreich. Der enddiastolische Druckgradient wurde von im Mittel 12±5 auf 4±3 mmHg reduziert. Die Nachuntersuchung bei 13 Patienten — in einem Fall nach einer zweiten Dilatation — nach 3–6 Monaten (n = 12) bzw. nach 1–11/2 Jahren (n = 5) bestätigte ein anhaltend gutes Ergebnis mit einem enddiastolischen Druckgradienten von im Mittel 4±3 mmHg.
