Current Gastroenterologic Aspects of Pernicious Anemia

  • Chapter
Folates and Cobalamins

Abstract

It is generally recognized that a patient suffering from atrophic gastritis of the fundic portion of the stomach becomes successively deficient in hydrochloric acid secretion, pepsin, and then intrinsic factor (IF). The atrophic gastritis observed in pernicious anemia causes failure of gastric secretion. The most direct consequences of this process include microbial overgrowth in the stomach (which is generally well tolerated), increased levels of serum gastrin secreted by the antrum (a relatively constant reaction to increased gastric pH when the antrum is spared), and vitamin B12 deficiency.
