Zusammenfassung
Die klinische Effektivität eines antimikrobiellen Chemotherapeutikums wird oftmals durch die direkte Korrelation der in-vitro-Wirksamkeit des Chemotherapeutikums zu seiner Pharmakokinetik ermittelt; unterschiedliche Modellvorstellungen, die verschiedene pharmakokinetische Parameter und auch die Serumeiweißbindung mit berücksichtigen, wurden zur Wertbemessung eines Chemotherapeutikums entwickelt (Zusammenfassung bei 16). Eine direkte Extrapolation von der in-vitro-Wirksamkeit eines Chemotherapeutikums auf dessen therapeutische Effektivität setzt unter anderem — trotz der Berücksichtigung der Pharmakokinetik des prüfenden Präparates im Makroorganismus — voraus, daß keine Interaktionen zwischen dem die Infektion verursachenden Mikroorganismus, der Infektabwehr und der Chemotherapie bestehen. Auf die Bedeutung des Immunsystems bei der Infektabwehr einerseits und die Beeinträchtigung der körpereigenen Infektionsabwehr durch Bakterien und deren Produkte andererseits wurde bereits vielfach hingewiesen (12). Weiterhin setzt die oben angesprochene direkte Extrapolation von in-vitro- und in-vivo-Charakteristika voraus, daß unter anderem die Physiologie, Virulenz und Pathogenität der Mikroorganismen unter in-vitro- und in-vivo-Testbedingungen identisch sind (Zusammenfassung bei 3).
