Zusammenfassung
Das wissenschaftliche Fundament zur Obstipation in der Schwangerschaft ist sehr begrenzt. Die wenigen verfügbaren Daten sind meist unbestätigt, unkontrolliert und vielfach auch methodisch anfechtbar. Als wesentlichste Teilkomponente in der komplexen Ätiologie der schwangerschaftsbedingten Obstipation gilt eine infolge erhöhten Progesteronspiegels ausgelöste, nach der Geburt schnell reversible, allgemeine Tonusverminderung der glatten Muskulatur, die im Gastrointestinaltrakt eine reduzierte Motorik zur Folge hat. Da ein erhöhter Progesteronspiegel aber bei allen Schwangeren besteht, müssen weitere Manifestationsfaktoren vorhanden sein, die nur unzureichend untersucht sind. Therapeutisch sollten nach differentialdiagnostischer Abklärung allgemeine und selten auch medikamentöse Behandlungsmaßnahmen erwogen werden. Die allgemeinen unterscheiden sich nicht von denen bei habitueller Obstipation. Medikamentös gilt nur der Einsatz von Füll- und Quellstoffen wie Weizenkleie und Psyllium als frei von fetalen Risiken. Ab dem 2. Trimenon sollte bei gelegentlichem Bedarf nur Folia Sennae, Phenolphthalein oder Bisacodyl verordnet werden. Während Paraffinöl, ganz besonders aber Rizinusöl und sonstige Anthrachinon-Derivate auch für den gelegentlichen Einsatz wegen potentiell größeren Schadens als Nutzen vermieden werden sollten, bleibt das potentielle fetale Risiko bei Einnahme der zahlreichen sonstigen Laxantien unbestimmt. Dies gilt auch für Laktulose.
