Abstract
From July 1984 to August 1987 we treated 44 patients with pure seminoma. Primary tumors were classified according to the WHO system. The β-HCG levels were determined using the RIA-gnost β-HCG kit by Behring.
Of our patients, 50% showed increased β-HCG levels before semicastration. Furthermore, we noted a tendency towards a rise in the frequency of increased β-HCG levels among our patients. We believe, however, that this effect is explained by the improvement of the radioimmunological method of determination, since no pathohistological changes were seen, i. e.; no increase in the relative number of syncytiotrophoblastic giant cells was shown.
Apart from two patients who had sceletal metastases and thus received chemotherapy, all our patients received radiation therapy. They have all remained in complete remission for a period of 2.5 years up to the present date. So far we have found no evidence of increased β-HCG levels influencing the prognosis of pure seminoma.
Zusammenfassung
Von Juli 1984 bis August 1987 behandelten wir 44 Patienten mit reinem Seminom. Die Klassifikation des Primärtumors erfolgte nach der WHO-Einteilung. Das β-HCG wurde bestimmt mit dem Testbesteck RIA-gnost β-HCG der Behringwerke Marburg.
50% unserer Kranken wiesen eine Erhöhung des β-HCG-Spiegels vor der Semikastratio auf. Es zeichnet sich in unserem Krankengut die Tendenz ab, daß reine Seminome mit β-HCG-Erhöhung im Zunehmen begriffen sind. Unseres Erachtens dürfte es sich hierbei um eine scheinbare Zunahme handeln, die durch die Verfeinerung der radioimmunologischen diagnostischen Methode bedingt ist, da das pathohistologische Korrelat, nämlich eine gleichzeitige Zunahme von syncytiotrophoblastären Riesenzellen fehlt. Unsere 44 Patienten wurden bis auf 2, die eine ossäre Metastasierung aufwiesen und chemotherapiert wurden, bestrahlt und sind über einen medianen Beobachtungszeitraum von 214 Jahren rezidivfrei.
Unsere eigenen Erfahrungen weisen darauf hin, daß der Nachweis des β-HCG beim reinen Seminom unabhängig vom Tumorausbreitungsstadium keinen Einfluß auf die Prognose der Erkrankung zu haben scheint.
