Abstract
The importance of risk factors in the early stages of NSGCT rises continuously. The high efficacy of polychemotherapy in advanced tumor stages justifies a “wait and see” strategy in early stages, depending on strict prognostic criteria. Until now treatment of stage I NSGCT was by retroperitoneal lymph node dissection and of seminoma by irradiation of paraaortic nodes. These therapies were well established and of low morbidity. Literature review and results of our own retrospective analysis showed vascular invasion and embryonal carcinoma as the most important prognostic factors in early stage NSGCT.
Zusammenfassung
Prognosefaktoren für den Hodentumor im frühen Stadium gewinnen zunehmend an Bedeutung. Die Ursache dafür liegt zum einen darin, daß durch die hohe Effektivität der Polychemotherapie der Tumor auch im Rückfall sanierbar ist und andererseits die Morbidität der Primärbehandlung daher auf ein Minimum reduziert werden kann. Bisher war in frühen Stadien nur die retroperitoneale Lymphadenektomie beim Nichtseminom und die Strahlentherapie der lokoregionären Lymphknotenfelder beim Seminom etabliert. Auf Grund der Literaturübersicht und unserer eigenen Daten ist zu schließen, daß die bedeutendsten Prognosefaktoren für den nichtseminomatösen Tumor (NSGCT) der Blutgefäßeinbruch (VI), das embryonale Karzinom (E) sowie das pT-Stadium des Primärtumors sind.
