Summary
In order to determine the relationship of restenosis following percutaneous transluminal angioplasty of peripheral arteries to the risk factors age, hypertension, lipidemic disturbances, diabetes, hyperuricemia, and smoking, as well as the attachment and spreading of thrombocytes, the number of circulating endothelial cells and euglobulin-lysis-time, we analyzed the clinical course of 77 peripheral angioplasties over 8.7 ± 3.1 months. Hypertension, diabetes, and hyperuricemia were significantly more frequent in the restenosis-group, but without differences in the respective serum levels. The frequency of lipidic disturbances and smoking was unexpectedly higher in the successfully treated group. A significant relationship was found between restenosis and attachment of thrombocytes to collagen, and the reduced number of circulating endothelial cells and prolonged euglobulin-lysis-time. Thrombocytes and endothelial cells are the primary target for risk factors, as well as for manipulation, and they are important in predicting restenosis.
Zusammenfassung
Mit dem Ziel der Erfassung eines Zusammenhanges zwischen Restenosierung und den Risikofaktoren Alter, Hypertonie, Fettstoffwechselstörungen, Diabetes mellitus, Hyperurikämie und Rauchen sowie der Thrombozytenhaftung, der Anzahl zirkulierender Endothelzellen und der Euglobulinlyse wurde der Krankheitsverlauf nach 77 Angioplastien der peripheren Arterien über einen Zeitraum von 8,7 ± 3,1 Monaten geprüft. Bei Patienten mit Restenosierungen waren Hypertonie, Diabetes mellitus und Hyperurikämie signifikant häufiger, ohne daß die initialen Blutspiegel differierten. Fettstoffwechselstörungen und Rauchen waren in der Gruppe ohne Restenosierungen häufiger. Ein signifikanter Zusammenhang bestand zwischen Restenosierung und gesteigerter Thrombozytenhaftung, verminderter Anzahl zirkulierender Endothelzellen sowie verlängerter Euglobulinlysezeit. Die zellulären Parameter sind von Bedeutung, da sie als Target von Manipulation und Risikofaktoren prädikti-ven Wert haben.
