Summary

Endarterectomy of the aorto-iliac axis has been performed more frequently in recent years in our patients. The 5-year cumulative patency rate according to Kaplan-Meier is 73.3% (results since 1980 included, n = 100). A meticulous operative technique is essential: A smooth proximal and distal transition (patchplasty) from the endarterectomy to the adjacent vessels, a smoothed inner surface, and intraoperative confirmation of a complete endarterectomy (angiography, angioscopy) determine the success of the procedure. Two modifications of the ringstripper and olive shaped curettes for finishing the inner surface are presented.

Zusammenfassung

Thrombendarteriektomien im aortoiliakalen Bereich werden in unserem Krankengut zunehmend häufiger durchgeführt. Unter Einschluß der Ergebnisse seit 1980 (n = 100) zeigen sich in der Kaplan/Meier-Kurve 5-Jahres Offenheitsraten von 73,3%. Entscheidend ist eine sorgfältige technische Durchführung mit einwandfreiem proximalen und distalen Übergang (Patchplastik) in die anschließenden Gefäße, eine Glättung der Gefäßinnenfläche sowie die intraoperative Kontrolle der Vollständigkeit der TEA (Angiographie, Angioskopie). Vorgestellt werden zwei technische Modifikationen des Ringdesobliterators sowie olivenförmige Schaber zur Nachbearbeitung der Gefäßinnenfläche.
