Plasma concentrations of atrial natriuretic peptide (α- h-ANP 1–28) during exercise and relationship to hemodynamic parameters in man

Summary

To elucidate the diagnostic value of Atrial Natriuretic Peptide (ANP), ANP plasma concentrations were determined by radioimmunoassay in 15 patients with coronary or valvular heart diseases who were to undergo right-heart catheterization during treadmill exercise. Continuous right atrial — (pRA), pulmonary artery — (pPA), pulmonary wedge pressures (pPC), and heart rate (HR) were correlated to ANP levels in pulmonary artery (PA), brachial artery (A), and peripheral vein (V); the best correlation was found between ANP (PA) and pPC with r=0.59 in linear regression analysis whereas no correlation was seen between HR and ANP values in PA, A and V during exercise.

With increased strain ANP concentration rose significantly in V, A and in PA (35 fmol/ml to 89 fmol/ml). ANP plasma levels were identical in A and PA, but were systematically lower in V compared to A and PA. During the first three minutes after starting treadmill exercise ANP increase in V was significantly (p<0.01) delayed, whereas ANP concentrations in A and PA rose immediately.

The data suggest that elevation of ANP during graded exertion is caused by an increase in atrial pressure and not by a rise in HR. Since there is only a quantitatively weak correlation between ANP plasma concentrations and pressures in the pulmonary circulation the predictive value of ANP levels in individual cases is limited.
