Abstract

A considerable amount of data has now been accumulated which clearly shows that the second heart sound is produced by diastolic vibration of the closed aortic and pulmonary valves (5, 6, 8). An understanding of the physical and physiological basis of production of the second heart sound has led to a better understanding of several auscultatory observations (8, 9), thereby permitting a more refined interpretation of some aspects of cardiac auscultation. Identification of the precise mechanism of production of the second heart sound has led to the prospective development of diagnostic techniques based upon the predicted behaviour of the heart sounds under various pathophysiological conditions. In particular, knowing that the stiffening of a structure raises its natural frequency enabled the prediction that the frequency of the second sound would increase as the valve stiffened due to disease. Analysis of the frequency content of the heart sound has confirmed this prediction (8, 10, 11) and has led to a diagnostic indicator of pathological stiffening of porcine bioprosthetic valves. This may serve as an early indicator of bioprosthetic valve degeneration.
