Zusammenfassung
Die Prognose von Patienten mit idiopathischer dilativer Kardiomyopathie (IDC) wird maßgeblich durch den Schweregrad der linksventrikulären Funktionsstörung bestimmt. Dem entspricht, daß die Mehrzahl der Patienten mit dieser Erkrankung schließlich am therapierefraktärem Pump versagen verstirbt. Zahlreiche Patienten (5, 8, 11) erreichen jedoch nicht ein solches Terminalstadium der Erkrankung, sondern versterben vorzeitig unter dem Bild eines plötzlichen Herztodes. Man hat lange spekuliert, inwieweit die bei der IDC außerordentlich häufigen und meist schweren ventrikulären Arrhythmien mit dieser Gefährdung in Zusammenhang stehen. In den letzten Jahren haben sich die Hinweise dafür verdichtet, daß, ähnlich wie bei der koronaren Herzkrankheit, auch bei dieser Erkrankung häufig auftretende, komplexe ventrikuläre Arrhythmien auf eine Gefährdung dieser Patienten durch einen plötzlichen Herztod hinweisen (3, 5, 8, 11, 18). Ein deutlich erhöhtes Risiko tragen dabei Patienten, die sowohl eine reduzierte links ventrikuläre Funktion als auch häufige ventrikuläre Paare oder ventrikuläre Tachykardien im Langzeit-EKG aufweisen (8). Ähnlich wie bei bestimmten Patientengruppen mit koronarer Herzkrankheit (6, 14, 19) und wie bei Patienten mit hypertropher Kardiomyopathie (4, 7) stellt sich auch hier die Frage, ob durch den Einsatz der programmierten Elektrostimulation als pro vokativdiagnostisches Verfahren gefährdete Patienten früher erkannt werden können. Wir haben daher bei 42 Patienten mit IDC die Inzidenz und prognostische Bedeutung elektrisch induzierter, ventrikulärer Arrhythmien prospektiv untersucht (9).
