Zusammenfassung
Kontraktions- und Förderleistung koronarkranker Herzen sind auch außerhalb des ischämischen Schmerzzustandes und im symptomfreien Intervall gestört. 17–35% der Koronarpatienten zeigen im Verlauf der Erkrankung eine manifeste Herzinsuffizienz (7). Aber auch ohne Herzinsuffizienz und ohne Herzvergrößerung ist das Herzzeitvolumen (HI, Herzindex), die mittlere systolische Austreibungsgeschwindigkeit (MSER) (6, 14, 16), sowie die Ejektionsfraktion (EF) (1, 2, 17, 18) in vielen Fällen schon bei Ruhe erniedrigt, während die intrakardialen Drucke normal sein können. Sozusagen immer ist die Anpassung an Belastung gestört: HI und MSER steigen nicht oder nur ungenügend an (2, 7, 13, 14), während der Füllungsdruck zunimmt (2, 13–16). Patienten mit niedrigem HI bei normalem Füllungsdruck und normalem Sauerstoffverbrauch hat Malmborg als hypokinetisch bezeichnet (14). Die kontraktilen Eigenschaften des koronarkranken Herzens werden bei Ruhe als normal oder leicht erniedrigt angegeben (5, 13) und nehmen bei Belastung nicht oder nur ungenügend zu (5).
