Zusammenfassung
Bei vielen entzündlichen Erkrankungen sowie bei der Sepsis findet sich eine massiv vermehrte Synthese von Stickstoffmonoxid (NO), dessen Wirkung im Entzündungsprozess kontrovers diskutiert wird. NO wird von dem Enzym NO-Synthase (NOS) bei der Umwandlung von L-Arginin zu Citrullin in unterschiedlichen Zellen synthetisiert. Hierbei wird zwischen der constitutiven (cNOS) und der induzierbaren (iNOS) Form der NOS unterschieden. Die cNOS ist vorwiegend im Endothel, in neuronalen Zellen und Blutplättchen konstitutiv exprimiert und mit geringer Menge an NO-Produktion unter anderem für die Vasoregulation verantwortlich. Demgegenüber spielt die iNOS bei Entzündungsvorgängen durch eine massive Produktion von NO eine wichtige Rolle. Proinflammatorische Zytokine wie IL-1, IL-6, TNF-α und IFN-γ stimulieren die iNOS-abhängige NO-Synthese in unterschiedlichen immunkompetenten Zellen [1].
