Zusammenfassung

Durch die moderne chirurgische Stapler-Technik kann heute den meisten Patienten mit Rektumkarzinom eine sphinktererhaltende Resektion angeboten werden. Bei Tumoren im unteren und mittleren Rektumdrittel ist die totale mesorektale Exzision allgemein akzeptierter Standard, um in der gesamten Zirkumferenz eine onkologische Radikalität zu erreichen. Nach einer solchen tiefen Anastomose leiden die Patienten üblicherweise in unterschiedlicher Intensität an einer schlechten Enddarmfunktion. Diese kann im Langzeitverlauf persistieren. Stuhldrang, häufige Durchfälle und gelegentliche Inkontinenz können aus dem Verlust der rektalen Reservoirfunktion resultieren.
