Zusammenfassung

Die kontinenzerhaltende Proktokolektomie mit ileopouch-analer Anastomose stellt heute das chirurgische Verfahren der Wahl für die meisten Colitis ulcerosa Patienten und für ausgewählte Fälle mit einer familiären Polyposis dar. Obwohl die operative Technik deutlich verbessert wurde, wird weiterhin über die optimale Pouch-Konfiguration oder den Erhalt der analen Übergangszone (Transitionalzone) kontrovers diskutiert. Ungeachtet dessen kann heute die Operation, trotz potentieller Früh-und Spätkomplikationen relativ sicher durchgeführt werden. Von den Spätkomplikationen benötigen häufig die Pouchauslaßstenose (5–16%), Fisteln (7%) und pathologische Veränderungen der analen Transitionalzone (2–3%) eine operative Korrektur.
