Zusammenfassung

Maligne Lymphome können isoliert im Mediastinum (Lazzarino et al. 1997) entstehen oder als mediastinale Beteiligung eines generalisierten lymphomatösen Prozesses in Erscheinung treten. Das Spektrum des mediastinalen Befalls erstreckt sich von zufällig diagnostizierten einzelnen Lymphknoten bis hin zu großen Mediastinaltumoren. Mediastinale Raumforderungen können asymptomatisch verlaufen oder von einer sehr vielfältigen Symptomatik begleitet sein. Sie entwickeln sich auf relativ engem Raum in unmittelbarer Umgebung lebenswichtiger Organe. Durch Kompression und Infiltration kann es zu Funktionsstörungen der Trachea, des Ösophagus, der Schilddrüse, der Nn. phrenici, der Lungen, des Herzens sowie der großen Gefäße kommen. Maligne Lymphome des Mediastinums führen nicht selten zu Pleura- und Perikardergüssen (Bruneau u. Rubin 1965; Ruckdeschel et al. 1975). Je nach der Dynamik des tumorösen Prozesses kann sich dabei das klinische Bild akut binnen weniger Tage entwickeln oder langsam über Monate entstehen. Eine Korrelation zwischen der Größe der mediastinalen Raumforderung und allgemeiner klinischer Beschwerden (B-Symptomatik) ist nicht gegeben. Mehr als 50% aller Mediastinaltumoren sind zum Zeitpunkt der Diagnose asymptomatisch (Silverman u. Sabiston 1980).
