Zusammenfassung
„The aim of premedication in infants and children is to establish mental and emotional relaxation, reduced sensory input and metabolic rate and antagonize adverse reactions of the autonomic nervous system“[18]. Diese Definition der Ziele der Prämedikation bei Säuglingen und Kleinkindern klang zum Zeitpunkt ihrer Formulierung durch Collins im Jahre 1976 überzeugend, und in den Ohren mancher Anästhesisten tut sie es heute noch. Aber sie hält nicht in allen Teilen einer kritischen Überprüfung stand. Sie setzt voraus, daß bei jedem Kind die Erlebnisse dieser präoperativen Phase so verarbeitet werden, daß immer ein Schaden für das Kind entsteht. Sie impliziert auch, daß alle Kinder von einer Reduktion des sensorischen Inputs profitieren und daß eine Antagonisierung adverser Reaktionen des autonomen Nervensystems nur durch eine Prämedikation erreicht werden kann. Alle 3 Voraussetzungen treffen nach heutigem Wissensstand nicht zu.
