Untersuchungen zur Anwendung der histologischen Schnittrandkontrolle bei der operativen Therapie verschiedener Präkanzerosen und Neoplasien der Haut

Zusammenfassung

Bei den Präkanzerosen Lentigo maligna und Morbus Bowen sowie bei Hautmalignomen wie spinozellulärem Karzinom, Dermatofibrosarkoma protuberans und malignem Histiozytom sind auch nach einer Exzision, welche durch histologische Querschnittsuntersuchung bestätigt im Gesunden erfolgte, Rezidivierungen in loco bekannt. Ursache hierfür sind klinisch nicht erkennbare Anteile des Tumors im Randbereich, welche meist asymmetrisch und nicht an der ganzen Zirkumferenz vorhanden sind. Um solche Ausläufer festzustellen, bedarf es einer histologischen Kontrolle des gesamten Schnittrandes.

Ein weiterer Vorteil der Schnittrandkontrolle liegt darin, daß man nicht mehr blindlings gesunde Haut zu opfern braucht, sondern die Veränderungen mit reduziertem Sicherheitsabstand exzidieren kann und nur, falls nötig, gezielt nachoperieren muß.
