Strategie der operativen Therapie des malignen Melanoms

Zusammenfassung

Beim Vorliegen eines malignen Melanoms stellt die radikale Tumorexzision eine vitale Indikation dar. Unser operationstaktisches Konzept berücksichtigt die heute allgemein übliche Einteilung in Melanome mit niedrigem und solchem mit hohem Metastasierungsrisiko. Dabei werden sowohl klinische als auch histologische Faktoren für die Therapieplanung herangezogen. Wie die Erfahrungen bei unseren in den Jahren von 1979 bis 1985 operativ behandelten Melanompatienten (N=477) zeigen, stellt die weite dreidimensionale Tumorexzision bei Tumoren mit geringem Metastasierungsrisiko die optimale Therapie dar. Wegen der zunehmenden Häufigkeit klinisch okkulter Lymphknotenmetastasen, muß mit ansteigender Infiltrationstiefe des malignen Melanoms, bei Tumoren mit hohem Metastasierungsrisiko, jedoch die zusätzliche prophylaktische regionale Lymphadenektomie möglichst als En bloc-Resektion gefordert werden.

Unser chirurgisches Vorgehen wird eingehend geschildert, wobei auch darauf hingewiesen wird, daß die Standard-Techniken bei entsprechenden Tumorlokalisationen und bei En-bloc-Resektionen häufig nicht unerheblich modifiziert werden müssen. Dabei muß aber stets die Radikalität der Lymphknotenentfernung gewährleistet sein.
