Zusammenfassung
Herabgesetzte myokardiale Kontraktilität ist Ursache der Herzinsuffizienz bei dila-tativer Kardiomyopathie [22, 34], sie kann selbst bei vollem Einsatz der heute zur Verfügung stehenden therapeutischen Prinzipien meist nur ungenügend behandelt werden. Es besteht daher ein gesteigertes Interesse an Medikamenten, die ihre positiv inotropen Wirkungen auf nichtglykosidischer Basis und dennoch mit den Vorteilen eines dopaminartigen Effektes vermitteln könnten [11, 44]. DPI 201-106 (Sandoz AG, Basel, Schweiz; Abb. 1) ist 1982 als eine Substanz vorgestellt worden, bei welcher eine kontraktilitätssteigernde mit einer negativ chronotropen, aktionspotentialverlängernden und koronardilatierenden Wirkung verbunden ist [50]. Obwohl die exakte Bestimmung des Wirkungsmechanismus von DPI 201-106 (DPI) noch aussteht, greift das Medikament offenbar aktiv in die Kinetik der Na-Kanäle ein und sensibilisiert die kontraktilen Proteine für Kalziumionen [14, 21, 49, 50]; die positiv inotropen Effekte kommen jedenfalls nicht durch Stimulation von Histamin- oder β-Rezeptoren noch durch Katecholaminfreisetzung oder Hemmung von Phosphodiesterase oder Natrium-Kalium-ATPase zustande. Seine orale Wirksamkeit ohne toxische Nebenwirkungen lassen diese Substanz als ein potentiell klinisch einsetzbares Kardiotonikum erscheinen. Diese Untersuchung hatte daher das Ziel, die hämodynamischen Akuteffekte des Medikamentes bei Patienten mit dilatativer Kardiomyopathie zu charakterisieren, und zwar im Vergleich zum Dobutamin (DOB) [61], dem eingeführten Referenzmedikament bei akuter Herzinsuffizienz [2, 16, 53].
