Zusammenfassung
Unter posthepatischer Cholestase versteht man die Abflußbehinderung der Galle durch obstruierende Prozesse im Bereich des Ductus hepaticus dexter bzw. sinister, Ductus hepaticus communis oder des Ductus choledochus bis hin zu seinem Eintritt ins Duodenum. Die Ursachen solcher Cholestasezustände sind in vielen Fällen Indikation zur chirurgischen Intervention mit dem Ziel der Beseitigung bzw. Umgehung des Abflußhindernisses. Ob und inwieweit derartige Operationen durch die begleitende Cholestase risikoreicher werden und in welchen Fällen eine präoperative biliäre Dekompression im Sinne einer konzertierten internistischchirurgischen Therapieoptimierung sinnvoll ist, wird im folgenden zu diskutieren sein.
