Zusammenfassung

Gelingt bei der ersten operativen Versorgung einer Leberruptur die Blutstillung ohne tiefgreifende Durchstechungen, ohne Zurücklassen avitaler Parenehymbereiehe und ohne äußere Kompressionstamponade, so heilen die meisten Leberverletzungen rasch und gut. Probleme im postoperativen Verlauf sind dann zumeist auf zusätzliche rganfunktionsstorungen im Rahmen einer Polytraumatisierung zurückzufuhren. In diesem Zusammenhang ist vorrangig der Unfallschock als Negativfaktor zu sehen, also Mikrozirkulationsstörungen und Gewebshypoxie als Folge des Blutvolumenmangels. Besonders auch die bei schockbedingten Lungenfunktiönsstorungen und bei Lungenkontusionen notwendige Beatmungstherapie erhöht schlieBlich wesenthch das Infektionsrisiko.
