Maligne Melanome im HNO-Bereich

Zusammenfassung

Im Kopf- und Halsbereich findet man die höchste Flächendichte bösartiger Pigmentgeschwülste. Nach Untersuchungen der Erlanger Studie (1967–1978) wird die Häufigkeit maligner Melanome im Kopf- und Halsbereich bei Männern mit 10% und bei Frauen mit 9,8% angegeben [8]. Bei einer Untersuchung von 660 Melanompatienten fand Conley eine Verteilung der Melanome im Gesicht mit 32,3%, Skalp 19,7%, Ohr 18,6%, Hals 13,6% und Melanome der Schleimhaut mit 8,9% [1]. Von Fitzpatrick werden Gesichtshautmelanome mit 43%, Skalp 12,6%, Ohr und postauriculäre Region 26,6%, Hals 16,5% und Schleimhautmelanome mit 10% angegeben [23]. Donnellan beschreibt 119 Hautmelanome des Kopf- und Halsgebietes und fand dabei in 31% der Fälle das superfiziell spreitende Melanom (SSM), welches sich bekanntermaßen in zunächst oberflächlich-horizontaler Wachstumsphase ausbreitet. Das sich von vornherein vertikal invasiv ausbreitende noduläre Melanom (NM) wurde in größerer Häufigkeit mit 49% gefunden. 19% der Fälle konnten dem Lentigo maligna-Melanom (LMM) zugeschrieben werden [22]. Das LMM entsteht nach jahrelangem horizontalen in situ-Wachstum aus einer Melanosis circumscripta praeblastomatosa (Dubreuilh) bzw. Lentigo maligna (Hutchinson). Bemerkenswert ist, daß hierbei besonders Jochbein und Wangenregion (also die am stärksten lichtexponierten Regionen) bei älteren Menschen befallen werden. Hals, Skalp und Wange sind nach Batsakis die häufigsten Hautlokalisationen maligner Melanome im Kopf-Halsbereich. Nase und Kinn werden seltener befallen [24]. 36 maligne Melanome des Ohres, von Sylven zusammengestellt, waren im Bereich der Concha, des Tragus, des Antitragus, des äußeren Gehörganges, der prä- und infraauriculären sowie retroauriculären Region lokalisiert [32]. Auch das Gebiet der Anthelix kann betroffen sein.
