Zusammenfassung

Die methodische Aufteilung eines erkrankten Menschen in eine psychische, somatische und soziale Dimension beruht darauf, daß wir für eine gleichzeitige Betrachtung aller Aspekte des einheitlichen Ganzen „erkrankter Mensch“ keine sprachlichen Möglichkeiten haben. Psychologische Risikofaktoren belasten jeden einzelnen unter genauer zu definierenden Bedingungen und sind bei den verschiedensten Erkrankungen mehr oder weniger für Auslösung, Verlauf und Gesundung wesentlich. Die Betrachtung des individuellen psychosoziosomatischen Ursachengefüges bleibt deshalb für den Einzelfall trotz aller groß angelegter Studien unerläßlich (Abb. 1). Damit sind die 3 Forschungsfelder einer anthropologischen Medizin bestimmt. Neben der methodisch bedingten Isolierung von einzelnen Variablen steht immer die dialektische Einbeziehung der jeweils nicht miterfaßten Merkmale. Unter psychischen Prodromen wird die psychische Gesamtverfassung im Vorfeld einer Erkrankung verstanden, wobei aus methodischen Gründen nie wirklich eindeutig geklärt werden kann, ob die psychischen Phänomene echte Vorläufer einer somatischen Erkrankung sind oder gleichzeitig als psychische Korrelate des somatischen Geschehens oder als Reaktion darauf auftreten. Dies gilt für die psychischen Risikofaktoren und Prodrome wie für alle übrigen Risikofaktoren der Koronarerkrankung.
