Zusammenfassung
In immunhistologischen Untersuchungen an über 100 kryo-konservierten Gewebeproben maligner und nicht-maligner Herkunft konnte die einzigartige Spezifität des monoklonalen GD3-Gangliosid-Antikörpers mAk R-24 für maligne Melanomzellen etabliert werden. Dieser Antikörper (Ak) reagierte mit allen Tumorproben von 21 Patienten mit primärem malignen Melanom und von 20 Patienten mit metastatischem malignen Melanom. Im Gegensatz zu den Gewebeproben beim primären malignen Melanom zeigten Gewebeproben von Patienten mit metastatischem malignen Melanom im mehreren Fällen eine heterogene Anfärbung, selbst bei Metastasen, die vom gleichen Patienten gewonnen wurden. Normale Melanozyten in Hautpräparaten reagierten nicht mit mAk R-24. Normalgewebe unterschiedlicher Herkunft (über 50 Gewebeproben) zeigten ebenfalls keine positive Anfärbung bis auf vereinzelte Zellen (< 1%) in der Nebenniere und im Bindegewebe.
Die Spezifität des monoklonalen Antikörpers mAk R-24 wurde in einer ersten klinischen Anwendung bei 3 Patienten bestätigt. Es kam zu einer Entzündungsreaktion im Tumorbereich. Nebenwirkungen dieser Antikörpergabe wurden nicht beobachtet.
Summary
More than 100 cryopreserved tissue probes of malignant and non-malignant origin were studied by indirect immunoperoxidase staining with monoclonal antibody mAk R-24. This antibody recognizes the disialoganglioside GD3. A striking specificity of this mAk R-24 for malignant melanoma tissue was established. This antibody reacted with all tumor probes of 22 patients with primary malignant melanoma and of 20 patients with metastatic malignant melanoma. More than 80% of tumor cells stained positive in the probes of primary maUgnant melanoma. In contrast melanoma cells in tissue sections of patients with metastatic malignant melanoma showed a more heterogenous staining pattern, even in different metastatic sites of the same patient. Normal melanocytes of human skin did not react with antibody R-24 as well as more than 50 normal human tissue samples of different origin. Only singular cells (< 1%) in the zona reticularis of the pararenal gland and in connective tissue gave positive staining.
The specificity of the mAk R-24 was confirmed in a first clinical application. A local inflammatory response at the tumor site was observed in all 3 patients who had received mAk R-24. No side effects of this mAk-application were observed.
