Abstract
Among the many different methods applied for the purpose of characterisation of vesico-ureteral reflux (VUR) in an individual patient, radiographic grading has been widely used. However, differences between grading systems inhibit comparison and understanding, voiding cystourethrography (VCU) is not yet standardised, and the impact of radiographic reflux grade on the risk of renal scarring is controversial. Radiographic reflux grade is in many papers considered as one of the important risk factors, in others as insignificant. Some of the most recent publications do not even mention radiographic reflux grade. In view of the importance of the answer to this argument for the general understanding of the pathogenesis of renal scarring as well as for the strategy of individual treatment, we have for this report analysed selected data from the International Reflux Study in Children (IRSC) — a prospective collaborative study with clearly defined uniform criteria for the grade of VUR as well as for renal scarring and with standardisation of VCU.
