Zusammenfassung
Eine Reihe von Befunden weist auf eine unterschiedliche Genese von Transmit-terstörungen beim Parkinson-Syndrom (P.S.) hin: 1. direkte Folgen der Degeneration katecholaminerger Neuronen (z.B. Substantia nigra, Locus coeruleus) und 2. andere, dadurch funktionell sekundär beeinflußte Störungen. Die erste Gruppe umfaßt primär dopaminerge nigrostriäre Nervenfasern und noradre-nerge Bahnen, die von Locus coeruleus und Medulla ausgehend eine Reihe von Gehirnarealen (Cortex, Hypothalamus, Striatum etc.) innervieren (Jenner et al. 1983). Der Nachweis extrem reduzierter Dopamin(DA)-Konzentrationen und Tyrosinhydroxylase(TH)-Aktivität in nigrostriären Gehirnarealen sowie von verringertem Noradrenalin (NA), TH und Dopamin-β-hydroxylase (DBH) im Locus coeruleus ist wahrscheinlich direkt vom Denervierungsgrad abhängig. Bei Ausfall präsynaptischer Aktivität besteht als wichtigster Reservemechanismus der kompensatorische Ausgleich zur reduzierten DA-Neurotransmission in einer „feedback-gesteuerten Aktivierung“ präsynaptischer Neuronen, welche sich durch eine im Vergleich zur DA-Konzentration relativ gering ausgeprägten Verringerung der Metabolitenkonzentration auszeichnet (Bernheimer et al. 1973; Bokobza et al. 1984).
