Zusammenfassung

Epidemiologie, klinische Forschung und biochemische Grundlagenforschung der letzten Jahre haben gezeigt, daß starkes Rauchen, Bluthochdruck und Fettstoff-wechselanomalien zu den wesentlichen Risikofaktoren der atherosklerotischen Gefaßkrankheiten gezählt werden müssen. Die Normalisierung oder BeScitigung solcher Risikofaktoren verringert die Häufigkeit kardiovaskulärer Todesfälle. Die diätetische Behandlung von Fettstoffwechselanomalien (Reduktion der Cholesterinzufuhr, Reduktion der Kalorienzufuhr bei Adipositas) und des Hypertonus (salzarme Kost) gilt als Eckpfeiler der Therapie und ist hinsichtlich ihrer Wirkung unumstritten. Hingegen ist bei der medikamentösen Therapie von Fettstoffwechselanomalien und des Hypertonus immer wieder die Frage diskutiert worden, ob diesbezügliche Interventionsmaßnahmen die Gesamtmortalität bzw. die Mortalität an kardiovaskulären Komplikationen zu senken vermögen. Besonders kontrovers wird der Effekt der Antihypertonika bei der primären Prävention des Hypertonus auf das Herzinfarktrisiko diskutiert. Hingegen ist durch verschiedene epidemiologische Studien gut belegt und deshalb allgemein anerkannt, daß Patienten mit Infarktanamnese durch Betarezeptorenblocker vor einem Reinfarkt besser geschützt sind als durch ein Plazebopräparat. In der Primärprävention des Herzinfarktes weisen einige epidemiologische Langzeitstudien auf eine Abnahme koronarer Herzkrankheiten unter intensiver antihyper-tensiver Medikation hin, andere Studien hingegen nicht.
