Zusammenfassung
Unter Herzinsuffizienz versteht man einen Zustand, bei dem das Herz trotz ausreichendem venösen Rückfluß nicht in der Lage ist, den unterschiedlichen metabolischen Bedürfnissen des Körpers gerecht zu werden. Das Herzzeitvolumen (die Fähigkeit des Herzens, Blut zu pumpen), ist im Verhältnis zum 02- und Nährstoffbedarf des Körpers zu gering. Da die verschiedenen Erscheinungen der Herzinsuffizienz sehr komplex und noch nicht vollständig verstanden sind, blieb diese willkürliche Definition nicht unwidersprochen. Häufig wird das erstmalige Auftreten von Belastungsdyspnoe bei Patienten mit linksventrikulären Erkrankungen als Zeichen des Beginns einer Herzinsuffizienz gewertet. Dieses Symptom ist jedoch nicht immer ganz verläßlich. Ein körperlich sehr aktiver Patient wird früher eine Belastungsdyspnoe verspüren als ein Patient mit überwiegend sitzender Beschäftigung. Andererseits kann eine untrainierte gesunde Person bei leichter, jedoch ungewohnter körperlicher Anstrengung bereits unter Atemnot leiden.
