Abstract

I think we now all agree that asymptomatic ischemia does exist. The following results have been achieved:

  1. 1.

    Even in symptomatic ischemic events, there is a preceding asymptomatic period.

  2. 2.

    If in the same patient some ischemic attacks are symptomatic and others are asymptomatic, it is mainly a matter of severity of ischemia.

  3. 3.

    There are some patients who never suffer angina in spite of repeated episodes of ischemia. In these patients the determining factors are not ischemia-related, but person-related, such as decreased sensibility to pain. Changes in endorphin metabolism and psychological factors also play major roles. Only among these patients are silent infarctions frequent.
