Zusammenfassung

Der Analkanal liegt zwischen dem Durchtritt des Enddarmes durch den Beckenboden und dem Anal-rand (Abb. 8.1 u. 8.2). Die Durchtrittsstelle (= Hiatus analis oder Anorektalring) wird vom Diaphragma pelvis begrenzt. Dieses besteht aus verschiedenen Muskelanteilen6:

  • dem oberen Teil des M. sphincter ani internus, der eine Fortsetzung und Verdickung der glatten Ring-muskulatur des Rektum darstellt,

  • dem oberen (tiefen) Teil des M. sphincter ani externus, der als quergestreifter Muskel den inneren Sphinktermuskel muffenartig umgibt und nach oben in den M. levator ani übergeht,

  • dem M. levator ani, der vorn und seitlich vom Os pubis und Os ischii sowie von der Fascia obturatoria entspringt. Seine vorderen medialen Anteile bilden die M. puborectalis-Schlinge, die von beiden Seiten her das untere Rektum nach dorsal umgreift. Sie knickt bei ihrer Kontraktion den Enddarm nach vorn ab und erfüllt damit eine wichtige Kontinenzfunktion. Hinten wird der M. levator ani durch die kleinen Kokzygeal-muskeln komplettiert.
