Zusammenfassung
Der Herzglykosidverbrauch pro 1000 Einwohner ist in Deutschland deutlich höher als in allen anderen Ländern Westeuropas und den USA [6] (Abb. 1). Das mag außer einer Reihe von anderen Ursachen darin begründet sein, daß unsere englischen Kollegen z. B. mehr Diuretika verordnen als wir und die Indikationen für die Herzglykosidbehandlung schärfer, d. h. restriktiver stellen. Es ist tatsächlich fragwürdig, Patienten mit einer Belastungsherzinsuffizienz, die erst bei größerer körperlicher Anstrengung auftritt, also Stadium II nach der New York Heart Association, mit Digitalis zu behandeln. Die körperliche Schonung ist bei diesen Patienten durchaus als Alternative, vielleicht sogar als Therapie der Wahl, zu diskutieren. Andererseits gibt es eine Reihe von Patienten, die Digitalis nicht tolerieren, sei es, daß Bradykardien, AV-Blockierungen oder Übelkeit selbst bei geringer Dosierung auftreten oder, was gar nicht so selten ist, daß die Herzglykosidbehandlung wirkungslos bleibt. In diesen Fällen muß auf andere Therapieformen ausgewichen werden.
