Zusammenfassung

Seit der Entdeckung von Wenckebach im Jahre 1914, daß Chinin bei einem Malariapatienten zu einer Unterdrückung von Herzarrhythmien führte, ist die Behandlung von Herzrhythmusstörungen eine Domäne der medikamentösen Therapie. Leider ist auch heute noch die medikamentöse antiarrhythmische Behandlung in einem Teil der Fälle erfolglos oder mit intolerablen Nebenwirkungen belastet. Hieraus ergibt sich die Notwendigkeit alternativer Therapieformen. Eine dieser alternativen Möglichkeiten stellt die chirurgische Therapie von Herzrhythmusstörungen dar. Ein chirurgisches Vorgehen wurde erst möglich, nachdem die bis dahin nur experimentell durchgeführte Technik der direkten Potentialableitungen von freigelegten Herzen auch intraoperativ bei Menschen angewandt werden konnten. Solche Untersuchungen erfolgten schon relativ früh insbesondere zur Klärung elektrophysiologischer Fragen als Grundlage der Interpretation des Oberflächen-EKGs beim Menschen (Groedel u. Borchardt 1948; McGregor 1950; Carouso et al. 1952; Brusca u. Magri 1956; Blount et al. 1957; Barbato et al. 1959; Wasserburger et al. 1959; Jouve et al. 1960; Durrer et al. 1964, 1966; Boineau et al. 1968). Praktische Bedeutung erhielt dieses Vorgehen zunächst für die intraoperative Lokalisation des His-Bündels, um bei der Operation angeborener Herzfehler eine Verletzung des spezifischen Leitungssystems zu vermeiden (Stuckey u. Hoffman 1962).
