Abstract
The acute hemodynamic response to vasodilator drugs in patients with congestive heart failure is well known. In response to administration of drugs that dilate both arterial and venous vascular beds there is an increase in cardiac output, a reduction in right atrial and left atrial pressure, a reduction in pulmonary arterial pressure, a modest reduction in arterial pressure, and little change in heart rate. This hemodynamic response has been reported during infusion of sodium nitroprusside (1), and after oral administration of hydralazine and isosorbide dinitrate (2), prazosin (3), trimazosin (4), or captopril (5). Despite the salutary acute hemodynamic response to these drugs, the major question is whether chronic administration of any of these agents.will result in improved exercise performance, reduced morbidity, and longer life expectancy.
